To date, the origin of SID, and especially whether there is an involvement of cerebellar pathways as well as the pyramidal tract, remains a matter of debate. Therefore, this study aims to shed light on structural networks associated with SID and to derive a data-driven model to predict SID in patients with PD and STN-DBS.Methods Randomised, double-blinded monopolar reviews determining SID thresholds were conducted in 25 patients with PD and STN-DBS. A fibre-based mapping approach, implementing the calculation of fibr-wise ORs for SID, was employed to identify the distributional pattern of SID in the STN’s vicinity. The ability of the data-driven model to classify stimulation volumes as ‘causing SID’ or ‘not causing SID’ was validated by calculating receiver operating characteristics (ROC) in an independent out-of-sample cohort comprising 14 patients with PD and STN-DBS.Results Local fibre-based stimulation maps showed an involvement of fibres running lateral and posteromedial to the STN in the pathogenesis of SID, independent of the investigated hemisphere. ROC analysis in the independent out-of-sample cohort resulted in a good fit of the data-driven model for both hemispheres (area under the curve (AUC)left=0.88, AUCright=0.88).Conclusions This study reveals an involvement of both, cerebello-thalamic fibres, as well as the pyramidal tract, in the pathogenesis of SID in STN-DBS. The results may impact future postoperative programming strategies to avoid SID in patients with PD and STN-DBS 001030860 536__ $$0G:(DE-HGF)POF4-5251$$a5251 - Multilevel Brain Organization and Variability (POF4-525)$$cPOF4-525$$fPOF IV$$x0 001030860 588__ $$aDataset connected to CrossRef, Journals: juser.fz-juelich.de 001030860 7001_ $$00000-0003-0749-3840$$aPetry-Schmelzer, Jan Niklas$$b1$$eCorresponding author 001030860 7001_ $$0P:(DE-HGF)0$$aHannemann, Jonathan H$$b2 001030860 7001_ $$0P:(DE-HGF)0$$aThies, Tabea$$b3 001030860 7001_ $$00000-0002-2194-5930$$aStrelow, Joshua N$$b4 001030860 7001_ $$0P:(DE-HGF)0$$aRubi-Fessen, Ilona$$b5 001030860 7001_ $$0P:(DE-HGF)0$$aQuinting, Jana$$b6 001030860 7001_ $$00000-0001-5851-9268$$aBaldermann, Juan Carlos$$b7 001030860 7001_ $$0P:(DE-HGF)0$$aMücke, Doris$$b8 001030860 7001_ $$0P:(DE-Juel1)131720$$aFink, Gereon R$$b9 001030860 7001_ $$0P:(DE-HGF)0$$aVisser-Vandewalle, Veerle$$b10 001030860 7001_ $$0P:(DE-HGF)0$$aDembek, Till A$$b11 001030860 7001_ $$0P:(DE-Juel1)131613$$aBarbe, Michael T$$b12 001030860 773__ $$0PERI:(DE-600)1480429-3$$a10.1136/jnnp-2024-333434$$gp. jnnp-2024-333434 -$$n3$$pjnnp-2024-333434 -$$tJournal of neurology, neurosurgery, and psychiatry$$v96$$x0022-3050$$y2025 001030860 8564_ $$uhttps://juser.fz-juelich.de/record/1030860/files/PDF.pdf$$yRestricted 001030860 8564_ $$uhttps://juser.fz-juelich.de/record/1030860/files/post-print.pdf$$yRestricted 001030860 909CO $$ooai:juser.fz-juelich.de:1030860$$pVDB 001030860 9101_ $$0I:(DE-588b)5008462-8$$6P:(DE-Juel1)131720$$aForschungszentrum Jülich$$b9$$kFZJ 001030860 9131_ $$0G:(DE-HGF)POF4-525$$1G:(DE-HGF)POF4-520$$2G:(DE-HGF)POF4-500$$3G:(DE-HGF)POF4$$4G:(DE-HGF)POF$$9G:(DE-HGF)POF4-5251$$aDE-HGF$$bKey Technologies$$lNatural, Artificial and Cognitive Information Processing$$vDecoding Brain Organization and Dysfunction$$x0 001030860 9141_ $$y2025 001030860 915__ $$0StatID:(DE-HGF)0430$$2StatID$$aNational-Konsortium$$d2023-08-24$$wger 001030860 915__ $$0StatID:(DE-HGF)0200$$2StatID$$aDBCoverage$$bSCOPUS$$d2023-08-24 001030860 915__ $$0StatID:(DE-HGF)0300$$2StatID$$aDBCoverage$$bMedline$$d2023-08-24 001030860 915__ $$0StatID:(DE-HGF)0320$$2StatID$$aDBCoverage$$bPubMed Central$$d2023-08-24 001030860 915__ $$0StatID:(DE-HGF)0199$$2StatID$$aDBCoverage$$bClarivate Analytics Master Journal List$$d2023-08-24 001030860 915__ $$0StatID:(DE-HGF)1050$$2StatID$$aDBCoverage$$bBIOSIS Previews$$d2023-08-24 001030860 915__ $$0StatID:(DE-HGF)0113$$2StatID$$aWoS$$bScience Citation Index Expanded$$d2023-08-24 001030860 915__ $$0StatID:(DE-HGF)0150$$2StatID$$aDBCoverage$$bWeb of Science Core Collection$$d2023-08-24 001030860 915__ $$0StatID:(DE-HGF)1030$$2StatID$$aDBCoverage$$bCurrent Contents -
