Abstract: Major depressive disorder is a complex condition with heterogenous aetiology and heterogenous symptomatic profiles. Patients with major depressive disorder exhibit unique combinations of symptoms, represented by a wide spectrum of depressive phenotypes (e.g. mood swings, sleep disturbances, motor dysfunction, fatigue, appetite loss, and cognitive impairment). Studies have proposed risk factors and identified correlates of depression based on brain, psychological, lifestyle, genetic, and environmental phenotypes. Recently, there has also been a growing interest in investigating the relationship between depression and immuno-metabolic biomarkers, to deepen the understanding of the potential biological mechanisms behind depression [1]. Nevertheless, present findings of association of depression with both brain features and immuno-metabolic biomarkers remain weak [2][3]. Furthermore, the heterogeneity of depressive symptoms complicates the discovery of unique biomarkers and the understanding of neurobiological substrates [1]. In this study, we propose that the association with biomarkers can be better characterised by disentangling specific dimensions of depressive phenotype (within the subclinical and clinical population). Using a data-driven approach, we identified the mood and energy dimensions of depressive symptoms in an adult population cohort. We then show that these depressive symptom dimensions have distinct patterns of associations with immuno-metabolic and multimodal brain biomarkers.
